About the Role:
As an Adult Hospital Social Worker, you will provide high-quality, person-centred social work support to adults, promoting independence, choice, dignity and wellbeing. You will work collaboratively with individuals, families, carers and multidisciplinary professionals to assess needs, manage risk and support positive outcomes.
Key Responsibilities:
- Use a strengths-based, person-centred approach to support adults to maximise independence, choice and wellbeing.
- Complete complex assessments using a range of health and social care assessment tools, including Care Act assessments and contributions to Continuing Healthcare (CHC) processes.
- Apply the Mental Capacity Act, including undertaking the two-stage capacity test and contributing to Best Interests decision-making.
- Identify and promote appropriate enablement, rehabilitation and reablement opportunities.
- Lead and complete safeguarding enquiries, including complex cases and whole-service concerns where required.
- Work effectively with health partners, voluntary and community organisations, families, carers and multidisciplinary teams.
- Undertake work relating to the Court of Protection, including applications for authorisation of deprivation of liberty in the community where required.
- Assess and manage risk appropriately, ensuring decisions are proportionate, evidence-based and focused on the individual's rights and wellbeing.
- Develop and review care and support plans in partnership with adults and relevant professionals.
- Maintain accurate, timely and reflective case records in accordance with professional and organisational standards.
- Contribute to service development, quality improvement and continuous improvement initiatives.
- Participate in reflective practice, supervision and ongoing professional development.
