Locum | Full-time | Hybrid
Luton Borough Council is looking for an experienced Adult Social Worker to join its Supporting Independence Team (SIT) on a locum basis.
The Supporting Independence Team works with adults to promote independence, prevent escalation of care needs and help people remain safely within their communities for as long as possible.
The role:
- Undertake Care Act assessments and reviews.
- Work with adults to identify their strengths, needs and desired outcomes.
- Develop person-centred support plans focused on promoting independence.
- Identify preventative and community-based support options.
- Work alongside health, voluntary and community services and other professionals.
- Safeguard adults where appropriate and respond to identified risks.
- Support people to access appropriate services and resources.
- Review outcomes and adapt support where circumstances change.
- Maintain accurate records and ensure work is completed in line with statutory requirements.
Requirements:
- Social Work England registration.
- Social Work qualification.
- Recent experience within Adult Social Care.
- Good working knowledge of the Care Act 2014, safeguarding and strengths-based practice.
- Ability to manage a caseload independently.
- Strong communication and partnership-working skills.
- Previous experience of working in a prevention, early-intervention, locality or community-based adult social care setting would be advantageous.
