About the role: We are recruiting an experienced Adults Social Worker to join the Community Review Team in Brent. This role focuses on reviewing care and support plans to ensure adults receive effective, person-centred services that promote independence and wellbeing.
Key Responsibilities:
- Complete Care Act reviews and reassessments
- Evaluate the effectiveness of care and support plans
- Work with adults with a range of needs, including older adults, physical disabilities and mental health
- Liaise with service users, carers, providers and partner agencies
- Identify risks, safeguarding concerns and required interventions
- Maintain accurate, timely case records
