Hospital Social Worker

We are seeking an experienced and motivated Social Worker to join our Hospital Discharge Team. The successful candidate will undertake person-centred assessments and reviews for Haringey residents, service users, and their carers in accordance with the Care Act 2014 .

Key responsibilities include:

• Completing Care Act assessments, reviews, and support planning.
• Promoting independence and wellbeing through a strengths-based approach.
• Identifying and implementing preventative interventions to reduce, delay, or avoid the need for ongoing support.
• Working collaboratively with health professionals, voluntary organisations, community groups, service users, and carers to achieve positive outcomes.
• Supporting safe and timely hospital discharges while ensuring statutory responsibilities are met.
• Managing a complex caseload within a fast-paced hospital environment.

Essential Requirements

• Qualified Social Worker with Social Work England registration.
• Significant experience of working within a hospital setting and supporting hospital discharge pathways.
• Sound knowledge of the Care Act 2014 , Mental Capacity Act, and safeguarding legislation.
• Experience of undertaking strengths-based assessments and person-centred support planning.
• Ability to work effectively within a multidisciplinary team and manage competing priorities.

Working Arrangements

This is a hybrid role , with an expectation that staff will work on-site at least three days per week , alongside flexible remote working arrangements where service needs allow.

Hospital experience is essential for this role and applications from candidates without recent hospital-based social work experience will not be considered.
