About Us:
Achieving for Children is seeking an experienced and committed Social Worker – Children with Disabilities to join its specialist Children with Disabilities service.
This is an excellent opportunity for a qualified Social Worker who is passionate about improving outcomes for children and young people with disabilities and supporting their families to access the right services and support.
You will work as part of a multi-disciplinary environment, undertaking assessments, managing risk and delivering effective social work interventions that promote the safety, wellbeing, development and independence of children and young people.
The Role:
- Undertake high-quality assessments of children and young people with disabilities and their families.
- Develop, implement and review child-centred plans that promote positive outcomes.
- Safeguard children and young people and respond appropriately to identified risks and concerns.
- Work collaboratively with parents, carers and children to understand their needs, wishes and aspirations.
- Work effectively with colleagues and partner agencies, including health, education and voluntary sector professionals.
- Provide appropriate social work interventions and support to children and families with complex needs.
- Manage a varied caseload effectively, prioritising work according to risk and need.
- Maintain accurate and timely case records in accordance with organisational policies and professional standards.
- Contribute to multi-disciplinary meetings, reviews and planning processes.
- Promote inclusion, participation, independence and positive outcomes for children and young people with disabilities.
- Ensure practice complies with relevant legislation, statutory guidance and professional standards.
