We’re seeking an experienced Hospital Social Worker to join a dynamic and supportive team in Wiltshire. This is an excellent opportunity to make a real difference by supporting adults during hospital discharge and transition back into the community.
Key Responsibilities
- Undertake assessments in line with statutory duties and best practice
- Determine appropriate care packages and support plans to meet individual needs
- Manage a large and complex caseload, ensuring timely intervention and safe discharge
- Maintain or improve the wellbeing and independence of clients through strengths-based approaches
- Work closely with hospital teams, community services, and families to coordinate care
- Professionally supervise and support other colleagues where required
- Safeguard vulnerable adults in accordance with the Care Act and other legislation
- Maintain accurate case records and contribute to service development
