We’re seeking an experienced Hospital Social Worker to join a dynamic and supportive team in Wiltshire. This is an excellent opportunity to make a real difference by supporting adults during hospital discharge and transition back into the community.

Key Responsibilities

  • Undertake assessments in line with statutory duties and best practice
  • Determine appropriate care packages and support plans to meet individual needs
  • Manage a large and complex caseload, ensuring timely intervention and safe discharge
  • Maintain or improve the wellbeing and independence of clients through strengths-based approaches
  • Work closely with hospital teams, community services, and families to coordinate care
  • Professionally supervise and support other colleagues where required
  • Safeguard vulnerable adults in accordance with the Care Act and other legislation
  • Maintain accurate case records and contribute to service development
