Mental Health Hospital Discharge Social Worker

Location: County Hall North, Horsham, West Sussex Hybrid: 3 days office / rest from home Pay: £35 ph + £150 weekly accommodation or £38ph only

The Role

We are looking for an experienced Social Worker to join our Mental Health Hospital Discharge Team. You will support adults aged 18+ who are ready to leave hospital, completing statutory social work duties and ensuring safe, effective discharge planning.

Key Duties

• Complete Care Act assessments, Mental Capacity Assessments, s117 aftercare planning, safeguarding and Best Interest Meetings. Manage a varied caseload and develop strengths-based, outcome-focused support plans. Work closely with the Mental Health Discharge Hub and partner agencies including Sussex Partnership NHS Foundation Trust. Cover countywide hospitals, potentially focusing on either 18–65 or 65+ age groups. Maintain compliance with legislation, policies and Social Work England standards. Work flexibly and travel independently across the county (driving licence preferred).

What We’re Looking For

• Qualified Social Worker with SWE registration. Experience in mental health or hospital discharge settings. Ability to work in a fast-paced environment, prioritise effectively and communicate professionally.
