About the Role
We are seeking a proactive and dynamic Family Wellbeing Practitioner to work within the Early Help service in London Borough of Merton. The role focuses on building strong, meaningful relationships with children, young people (0–25), adults, and families to deliver high-quality whole-family assessments and coordinated multi-agency support.
You will work as a Lead Professional for a caseload of approximately 12–15 families with multiple and complex needs, supporting improved outcomes and reducing risks that may lead to statutory intervention.
Key Responsibilities
- Undertake comprehensive whole-family needs assessments and co-produce tailored support plans with families
- Act as Lead Professional, coordinating multi-agency interventions across children’s and adult services
- Deliver intensive, relationship-based practice with families facing complex challenges
- Support adults in overcoming barriers such as:
- Substance misuse
- Mental health needs
- Domestic abuse
- Offending behaviour
- Housing instability
- Support pathways into employment and improved parental capacity.
- Reduce risks linked to:
- School non-attendance
- Developmental delay
- Safeguarding concerns
- High-risk behaviour
- Work within the Early Help Practice Model
- Apply the Signs of Safety to all assessments and interventions
- Lead and coordinate effective multi-agency network meetings and communication
- Monitor and record progress against service standards and family outcomes
Essential Requirements
- Experience working with children, young people, and families with complex needs
- Strong understanding of safeguarding and risk management
- Ability to coordinate multi-agency services effectively
- Experience delivering strengths-based, outcome-focused interventions
- Confident working independently with a complex caseload
- Strong communication, assessment, and case management skills
