EHC Coordinator (Preparation for Adulthood)

You will be at the heart of supporting young people in our vibrant borough as they navigate the transition from school or college into adult life. In this role, you aren’t just managing cases; you are ensuring that Education, Health, and Care Plans (EHCPs) reflect real aspirations.

Duties

  • Transition Planning: Attend and contribute to annual reviews from Year 9 onwards, ensuring PfA outcomes—such as employment, independent living, and community participation—are embedded.
  • Multi-Agency Collaboration: Work closely with families, schools, colleges, social care, and health professionals to ensure smooth transitions into adult services.
  • Creating Opportunities: Promote access to supported internships, apprenticeships, and vocational training.
  • Clear Communication: Act as a bridge between complex statutory processes and families, explaining information sensitively.
