About the Role
An exciting opportunity has arisen for an experienced Adults Social Worker to join a busy Community Team in North Essex.
The successful candidate will work as part of a supportive adult social care service, helping adults to maintain their independence, improve their wellbeing and achieve positive outcomes. You will manage a varied caseload and undertake assessments, reviews and care planning in line with statutory requirements.
This is an excellent opportunity for a skilled Social Worker who is confident working autonomously and is experienced in delivering strengths-based, person-centred support.
Key Responsibilities
- Manage a varied caseload of adults with a range of care and support needs.
- Undertake Care Act assessments, reviews and reassessments.
- Develop person-centred care and support plans that promote independence and wellbeing.
- Identify strengths, risks and eligible care and support needs.
- Safeguard adults at risk and respond appropriately to safeguarding concerns.
- Work collaboratively with individuals, families, carers and partner agencies.
- Coordinate appropriate services and support packages to meet identified needs.
- Maintain accurate and timely case records in accordance with statutory and organisational requirements.
- Manage workload effectively and ensure assessments, reviews and interventions are completed within required timescales.
