About the role
This role is centered around a Home First / Discharge to Assess approach, helping individuals achieve the best possible outcomes by supporting their return home wherever appropriate. Working as part of a successful partnership between health services, adult social care, and the Home Independence and Crisis Support (HICS) team, you will play a key role in facilitating safe and timely hospital discharges.
Previous experience in hospital discharge services is highly desirable. You will need strong knowledge of adult social care legislation, strengths-based practice, safeguarding, the Mental Capacity Act, and Court of Protection processes.
Key Responsibilities
- Lead on strengths-based assessments and complex hospital discharge planning to support individuals returning home safely and promptly.
- Undertake risk assessments, care planning, and reviews in line with the Care Act 2014 and Home First / Discharge to Assess principles.
- Work collaboratively with NHS colleagues, HICS teams, GPs, housing providers, and other partner agencies to coordinate effective support and discharge arrangements.
- Apply the Mental Capacity Act 2005, undertake Best Interests decision-making, and contribute to Court of Protection work where required.
- Identify and respond to safeguarding concerns, ensuring the safety and wellbeing of vulnerable adults.
- Manage a complex caseload, attend hospital and community visits across Dorset, and participate in office-based duty support as required.
