We are currently recruiting for two Occupational Therapists to join a dynamic and supportive team delivering care across the Discharge to Assess (D2A) pathways in the Basildon, Broomfield, and Southend areas. This is an excellent opportunity to make a meaningful impact by helping patients transition safely from hospital to home or community settings.
Key Responsibilities:
- Undertake holistic assessments and interventions in community and care home settings
- Identify and address therapy and care needs to reduce hospital re-admissions and promote independence
- Develop, implement, and evaluate person-centred care and therapy plans
- Provide timely equipment assessments and support discharge planning
- Set rehabilitation goals to enhance function and quality of life
- Work autonomously within professional and organisational guidelines
- Coordinate with multi-disciplinary teams to ensure the most appropriate pathway for each patient
Requirements:
- HCPC-registered Occupational Therapist
- Valid UK driving licence and access to own vehicle with appropriate business insurance
- Willingness to travel across Mid and South Essex
