The Hospital Social Worker Team in West Sussex requires an experienced Social Worker to join their team.
Location: St Richards Hospital, East Surrey Hospital
Working Arrangement: Hybrid - 2-3 days per week based at the hospital
Pay: £38 per hour OR £35 per hour + £150 per week expenses
About the Role:
We are looking for an experienced and motivated Social Worker to join the Hospital Social Work Team at St Richards Hospital and make a positive difference to the lives of people across West Sussex. This is a varied and fast-paced role, supporting individuals during and following their hospital admission. You will undertake assessments, coordinate care and support, and work with individuals, families, carers and professionals to help people regain their independence and achieve the best possible outcomes. The role involves a mixture of home and hospital-based working, with a requirement to attend the hospital 2-3 days per week. You will visit wards to undertake assessments, attend family meetings and support the wider hospital social work function. You will also work as part of the Multi-Disciplinary Team (MDT) covering A&E and the Emergency Floor.
Key Responsibilities:
- Deliver a statutory social work service in accordance with relevant legislation, policies and professional standards.
- Complete timely, accurate and strengths-based assessments of individuals' needs.
- Develop and implement effective care and support plans focused on achieving positive outcomes.
- Support people to regain skills, independence and confidence following hospital admission wherever possible.
- Work collaboratively with individuals, families, carers and a wide range of professionals and external agencies.
- Undertake work relating to safeguarding enquiries where required.
- Apply a risk-enablement approach and evidence-based practice to decision-making.
- Analyse and interpret information to identify and prioritise needs and determine appropriate interventions.
- Attend ward assessments, family meetings and MDT discussions.
- Work closely with colleagues across hospital and community services, including voluntary-sector organisations.
- Maintain accurate and confidential records in line with data protection requirements.
- Provide informal supervision, mentoring and coaching to less experienced practitioners, students and apprentices where appropriate.
- Recognise the limits of your own accountability and seek appropriate advice, support and supervision when required.
