We are recruiting a skilled and compassionate Social Worker to join the Hospital Social Work Team in West Lothian. This is a rewarding opportunity to work within a supportive, experienced, and multidisciplinary hospital-based team, helping adults and their families during times of significant change.
The Role
- Complete strengths-based assessments to support safe and timely hospital discharge
- Work closely with health colleagues across wards and community services
- Support adults and their carers to make informed decisions about care and support
- Undertake capacity, risk, and safeguarding assessments where required
- Contribute to adult protection work and complex decision-making
- Maintain accurate, high-quality case records and reports

- Degree or equivalent qualification in Social Work
