Adult Hospital Team in Barnsley requires an Advanced Practitioner. Immediate start and on-going contract. Paying £37 per hour. We are seeking an experienced and motivated Advanced Practitioner to join our Adults Hospital Discharge Team in Barnsley. This is a key leadership role, supporting the delivery of timely, safe and person-centred discharges from hospital while ensuring high standards of professional practice.
You will work closely with NHS colleagues and partner agencies to manage complex cases, provide expert guidance to practitioners, and promote strengths-based, lawful decision-making in line with the Care Act, Mental Capacity Act and safeguarding legislation. This role offers the opportunity to influence practice, support staff development and contribute to improved outcomes for adults and their families.
Responsibilities- Lead complex hospital discharge cases and high-risk decision-making
- Complete and quality-assure Care Act and MCA assessments
- Provide professional guidance, oversight and supervision to staff
- Lead safeguarding activity and multi-agency working
- Support timely, lawful and person-centred hospital discharges
- Work closely with NHS and partner agencies to reduce delays
- Ensure compliance with legislation, policy and best practice
