The Role As an Adults Social Worker within the Complex Care Team, you will: * Manage a varied and complex caseload * Complete Care Act assessments and reviews * Develop person-centred care and support plans * Assess and manage risk effectively * Work with service users, families and carers * Promote independence, choice and wellbeing * Work closely with health and other partner agencies * Undertake safeguarding work where required * Maintain accurate and up-to-date case records About You The successful candidate will have: * A recognised Social Work qualification * Current Social Work England registration * Experience working within Adult Social Care * Experience managing complex cases * Strong assessment and care planning skills * Excellent communication and organisational skills * The ability to work independently and as part of a wider team Interested?
