Qualified Social Worker needed for our Community Mental Health Team (Older Adults) in Bracknell, paying £37.90 ph - Hybrid Working.
The role is in our Community Mental Health Team for Older Adults. This is an opportunity to support older individuals to live as independently and confidently as possible within their communities. Our team champions a strength-based approach, placing each individual’s independence at the centre of our practice. We are committed to empowering people, promoting dignity, and ensuring that even the most vulnerable residents receive the support they need at the right time.
Key Responsibilities
- Build meaningful relationships and take time to truly understand the individuals you support
- Deliver person-centred assessments, care planning, and interventions
- Work collaboratively with health professionals, families, and partner organisations

- Qualified Social Worker with relevant registration
