Tripod Partners are currently recruiting for an experienced Adults Social Worker to join the Adult Early Support Team at Manchester City Council. This is an exciting opportunity to join a team focused on providing early intervention and support to adults, helping people maximise their independence and achieve positive outcomes.
The Role
- Manage a caseload of adults requiring support
- Complete Care Act assessments and reviews
- Identify individuals' strengths, needs and desired outcomes
- Develop person-centred support plans
- Promote independence and prevent escalation of need
- Work collaboratively with individuals, families and carers
- Liaise with health and other partner agencies
- Assess and manage risk
- Undertake safeguarding responsibilities where required
- Maintain accurate case records and documentation
About You
- A recognised Social Work qualification
- Current Social Work England registration
- Experience working within Adult Social Care
- Strong assessment and care planning experience
- A person-centred approach
- Excellent communication and organisational skills
- The ability to work effectively
