About The Role:
- Carry out Care Act assessments and reviews within the community
- Support adults with identifying needs, strengths, and appropriate support plans
- Work collaboratively with service users, families, carers, and partner agencies
- Promote independence and person-centred practice
- Maintain accurate and timely case records and assessments
- Travel across rural areas within Lewes and North Wealden to undertake community visits
- Due to the nature of the role, the successful candidate must be a car user.
Key Responsibilities:
- Be a qualified and registered Social Worker
- Have at least 2 years’ recent experience working within a similar Adult Social Care or community-based team
- Have experience completing Care Act assessments and reviews
- Be confident managing community-based caseloads independently
- Possess strong communication and assessment skills
- Be organized, adaptable, and committed to delivering high-quality support
