Social Worker - Integrated Discharge Team - North Lincolnshire Council - £35per hour
The Integrated Discharge Team is looking for a social worker to work a shift pattern over 7 days to complete Discharge Facilitation forms as part of the MDT team at Scunthorpe General Hospital.
Completed discharge facilitation forms.
Have an understanding of the care act, mental capacity act, chc checklists and how to commission services.
