Adult's Social Worker needed for our Hospital Team in Bournemouth paying £32.50 ph!

- Rate: £32.50per hour
- Working pattern: Hybrid - candidates will be required to work in the office some of the time
We are seeking an experienced Adult Social Worker to join the Hospital Social Work Team based at University Hospitals Dorset.
The successful candidate will ideally have recent hospital social work experience and a strong understanding of Discharge to Assess (D2A) pathways. You will play a key role in supporting adults with complex needs to achieve safe, appropriate and timely discharge from hospital.
Key Responsibilities
- Work collaboratively within a hospital environment to support safe and timely discharges
- Apply Discharge to Assess principles to ensure effective patient flow
- Liaise with multidisciplinary teams, patients, families, and external agencies
- Ensure assessments and decisions are person-centred and outcomes focused
- Maintain accurate and timely records in line with policies and procedures
Key Responsibilities
- Undertake strengths-based assessments and identify individuals' care and support needs.
- Support complex discharge planning in partnership with patients, families and professionals.
- Complete Care Act assessments and reviews.
- Undertake safeguarding enquiries and respond appropriately to concerns.
- Complete Mental Capacity Act assessments and contribute to best-interest decision-making.
- Work effectively as part of a multi-disciplinary team (MDT).
- Support effective Discharge to Assess pathways and promote independence wherever possible.
- Liaise with health, social care and other partner agencies to facilitate safe and timely hospital discharge.
- Maintain accurate records and ensure work is undertaken in line with relevant legislation, policies and professional standards.
About You
- Current registration with Social Work England.
- Recent experience within Adult Social Care.
- Experience of working within a hospital or acute setting is highly desirable.
- Good knowledge and understanding of hospital discharge pathways and Discharge to Assess.
- Experience of Care Act assessments, safeguarding and the Mental Capacity Act.
- Excellent communication, assessment and decision-making skills.
- The ability to work effectively within a fast-paced, multi-disciplinary environment.
- A strengths-based and person-centred approach to social work practice.
