Social Worker - Project Team - East Riding Council - £31.13 per hour
The 1-1 Project will support people living in a residential care home setting. The focus will be to support the safety and wellbeing of the residents and to ensure that any 1-1 hours are proportionate and evidenced to support the least restrictive care provision for the resident. The role works closely with the Review Team, Contract and Quality Assurance and Commissioning Teams. A working knowledge of both Safeguarding and DoLS legislation is required. Minimum of 2 years’ experience working as a qualified social worker in Adult Social Care and experience of undertaking Care Act 2014 care and support assessments and completing care and support plans is essential.
The role of Social Worker must be able to work with Adults as an autonomous professional, demonstrating expert and effective practice in complex situations. This will include supporting the team to undertake dynamic Care Act (2014) care and support review and assessments, updating support plans and risk assessments and review residential packages for Adult’s funded by East Riding of Yorkshire Council using a strengths-based approach to ensure that the care provided continues to meet people’s needs. A wide range of experience across Adult Services including Care Act Assessments and Support Planning, Mental Capacity Assessment and Best Interest Decisions. It is required that all meetings take place in person to ensure a robust review is completed. Hourly rates are clearly defined on the vacancy request, these will not be negotiated, so only put forward candidates who are available at these rates.
