Urgent Community Response Practitioner

Location: Teddington Pay Rate: £29.02 per hour Contract: 3 months Schedule: Monday to Friday Tripod Partners are seeking a Band 6 Urgent Community Response Practitioner to support a dynamic integrated care service that works across health and social care. This role is open to registered Nurses, Paramedics, or Therapists with assessment experience and a commitment to preventing unnecessary hospital admissions. Key Responsibilities: * Deliver rapid response assessments and interventions in patients\’ homes or care settings within 2 hours of referral * Support patients with complex needs to remain safely at home * Carry out risk assessments and develop individualised care plans * Work within a multi-disciplinary team and across professional boundaries * Identify deteriorating health conditions and escalate as appropriate * Maintain clinical documentation and support service audits and reviews * Participate in 7-day rota including occasional weekend shifts Essential Requirements: * NMC or HCPC registration * Minimum of 2 years post-registration experience * Strong assessment and clinical decision-making skills * Experience in community or acute care settings * Awareness of admission avoidance pathways * Ability to work autonomously and within a team Desirable: * Advanced assessment qualification * Experience in urgent community response, A&E, ambulance or reablement services * Venepuncture skills (or willingness to train) This role requires the right to work in the UK
