The Role

As a Rehabilitation Support Worker, you'll manage a caseload of participants within the community, providing personalised support from initial assessment through to the completion of their sentence. Working closely with each individual, you'll identify barriers, develop tailored action plans, and provide practical guidance to help them achieve their goals. Through one-to-one support, mentoring, and group-based interventions, you'll encourage positive behavioural change, improve wellbeing, and help participants build the confidence and skills needed to move forward.
