The predicted salary is between 19700 - 29200 £ per year.

To provide early and targeted support to aid the recovery and resilience of people with multiple vulnerabilities. You will work across a portfolio of longer-term accommodation-based services aimed at addressing the 'revolving door' nature of shorter-term supported accommodation. Demonstrating a commitment to safeguarding, you will support customers to develop the skills they need to achieve their goals, empowering them to build successful relationships within their community, access necessary services, and develop skills to better manage their health and wellbeing, increasing independence as they progress through a prevention pathway model.

We seek individuals with relevant experience in providing specialist support, covering activities such as:

  • Excellent communication skills and a customer-focused approach
  • Proven ability to work effectively with a wide range of stakeholders, statutory and non-statutory
  • Experience supporting a caseload across multiple locations in an agile working environment
  • Experience facilitating customer involvement
  • IT literacy, including Word, Excel, Support Planning Software, and database management
  • Maintaining accurate records to capture outcomes and KPIs
  • Ability to manage competing priorities and meet deadlines
  • Flexibility to work within a shift pattern environment
