Trident Healthcare Solutions is seeking an experienced Band 6 Occupational Therapist for a locum assignment based within the Intermediate Care Team at an NHS Trust in Hull. The successful candidate will independently manage a caseload of adult patients.
Responsibilities:
- Manage a caseload of intermediate care patients
- Complete assessments, treatment plans, discharge planning and onward referrals
- Ideally hospital discharge/Intermediate care/discharge to assess experience
- Maintain accurate and timely patient records
Skills and Experience Required:
- Experienced working within the NHS (or similar)
- SystmOne experience desirable
- Excellent written and verbal communication skills, including phone and email
Essential Requirements:
- Candidates must have NHS experience
- Candidates must hold HCPC registration
- Candidates must hold the right to work in the UK
- Must have a car as potential for travelling to other sites and patients’ homes
