Trident Healthcare Solutions is seeking an experienced Band 6 Physiotherapist to join an Intermediate Care service based in Hull. The successful candidate will support discharge pathways (including DT2A) and manage a varied caseload across multiple community sites and patient homes. A car is essential for this role.
Responsibilities
- Provide high‑quality physiotherapy within Intermediate Care settings
- Support safe and effective discharge planning, including DT2A pathways
- Manage a caseload across community sites and patient homes
- Complete timely and accurate clinical documentation
- Work collaboratively within a multidisciplinary team
- Utilise SystmOne (desirable) for clinical record‑keeping
Skills and Experience Required
- Degree‑qualified Physiotherapist
- Experience within Intermediate Care, community rehab, or discharge services
- Strong clinical reasoning and caseload management skills
- Excellent communication skills
Essential Requirements
- Previous NHS experience
- HCPC registration
- Car driver with access to own vehicle
- Experience in discharge pathways / DT2A preferred
