Trident Healthcare have an ongoing need for compassionate, motivated, and skilled Community Nurses\' to join a dedicated community nursing team in Sussex. This role offers the opportunity to deliver high-quality, person-centred care to patients in their own homes and community settings, supporting independence, rehabilitation, and chronic condition management. You will be part of a multidisciplinary team, playing a key role in the delivery of holistic nursing care and supporting patients to live safely and comfortably in their communities. Key Responsibilities: * Deliver skilled, evidence-based nursing care to patients in their homes or community clinics * Perform wound care, catheter and continence management, medication administration (including insulin and injectable therapies), and palliative/end-of-life care * Assess, plan, implement and evaluate individualised care plans in partnership with patients, carers, and families * Promote self-care and independence where appropriate * Collaborate closely with GPs, social care teams, physiotherapists, occupational therapists, and other allied health professionals * Maintain accurate and timely clinical records in line with NMC and local standards * Use clinical judgement and escalate concerns appropriately to senior colleagues or other professionals * Support junior staff, students, and healthcare assistants as required * Contribute to service development, quality improvement initiatives, and patient safety strategies Essential Requirements: * NMC Registered Nurse (Adult) * Strong communication and interpersonal skills * Full UK driving licence and access to a vehicle (essential for community visits) * Ability to work autonomously and as part of a team * Knowledge of safeguarding, infection prevention, and health promotion principles
