What you'll be doing:

  • Undertake effective needs and risk assessments with people referred to the service.
  • Deliver a flexible service, adjusting support frequency and intensity and re‑introducing support to dormant cases should tenancy or support issues arise.
  • Support customers to sustain accommodation, improve health and well‑being, develop skills to live independently; provide practical support, maximise income, budgeting and address debt.
  • Act as a community navigator, linking customers into local services and activities to establish networks of support and reduce social isolation.
  • Work with key agencies such as housing, health and substance misuse services and act as an advocate, unblocking barriers to facilitate access to services.
  • Adopt effective methods to consult and involve service users in the planning and delivery of the service they receive.
  • Promote traditional values, demonstrate pride in the work and service delivery.

Key qualifications and experience:

  • Experience of agile working across locations to support a case load of customers (desirable).
  • Excellent communication skills.
  • Proficient in using IT packages to capture all elements of support and produce databases and reports.
  • Relevant qualification specific to the role (desirable).
  • Understanding of the health and social care needs of the people receiving the service.
  • Working knowledge of legislation in regard to delivering support to young people.
  • Ability to develop effective working relationships with colleagues and professionals within partner agencies.
