About the role

As a Lead Worker, you will provide early, targeted, and flexible support to individuals and families across Telford and Wrekin.

You will work with young people aged 16-54 and older individuals aged 55+, supporting them to maintain their tenancies, achieve financial independence, and improve their health and well-being.

This role focuses on homelessness prevention, tenancy sustainment, and promoting independent living.

  • What you’ll be doing
  • Conduct comprehensive assessments of customers’ housing, financial, health, and social support needs.
  • Develop personalised, outcome-focused support plans tailored to individual strengths and goals.
  • Collaborate with multi-agency partners to deliver coordinated and holistic support services.
  • Provide practical assistance with budgeting, debt management, and accessing welfare benefits.
  • Support customers in connecting with community services, reducing isolation, and fostering social inclusion.
  • Uphold safeguarding standards, ensuring the health and safety of vulnerable individuals.
  • Maintain accurate records, ensuring compliance with Key Performance Indicators (KPIs).
