About the role
To provide early and targeted support to aid the recovery and resilience of single adults or childless couples with multiple vulnerabilities, working flexibly across the city supporting people in their own homes within supported housing settings, or in locations of their choice.
What you’ll be doing
- Undertake effective needs and risk assessments with people referred to the service in accordance with the service’s eligibility criteria.
- Deliver a flexible service, increasing or reducing the frequency and intensity of support as required and re-introduce support to dormant cases should any tenancy or support issues arise.
- Support customers to sustain accommodation, improve health and well-being outcomes, develop skills required to live independently, including offering practical support, maximising income, budgeting, addressing debt.
- Act as a community navigator, linking customers into local services and activities to establish networks of support and reduce social isolation.
- Work in collaboration with key agencies such as housing, health and substance misuse services and act as an advocate, unblocking barriers to facilitate access to services.
- Adopt effective methods to consult and involve service users in the planning and delivery of the service they receive.
- Proactively approach the promotion of traditional values, demonstrating pride in the work and service delivery.
Who we’re looking for
- Experience of agile working across locations to support a case load of customers is desirable but not essential.
- Excellent communication skills.
- Proficient in using IT packages to capture all elements of support provided and producing databases and reports when required.
- A relevant qualification specific to the role is desirable but not essential.
- An understanding of the health and social care needs of the people receiving the service.
- A working knowledge of legislation in regard to delivering support to young people.
- The ability to develop effective working relationships with colleagues and professionals within partner agencies.
