About The Role
Are you passionate about enhancing the well-being of citizens and helping individuals achieve stability and independence? We are seeking a dedicated Lead Worker to join our innovative accommodation-based health & wellbeing support service. There are 10 sites across Birmingham.
This role focuses on providing person-centred, strength-based support to adults facing multiple challenges, ensuring they access the resources and services they need to thrive. You will work within a holistic and preventative framework that prioritizes community integration, early intervention, and collaboration.
About The Service
Our Health and Wellbeing Accommodation Service supports childless adults aged 25+ (including those aged 65+) who have faced multiple barriers to independence. These may include:
- Substance use
- Mental health challenges
- Repeat homelessness
- Offending history
- Exclusion or risk of exclusion from mainstream services
The service operates through referral channels, working closely with Birmingham City Council and community-based advisors to provide strength-based, preventative support.
Key Responsibilities
- Deliver high-quality, personalised support tailored to individuals’ unique needs and circumstances.
- Facilitate access to community resources, advice, and guidance to empower informed decision-making.
- Collaborate with multi-disciplinary teams and referral agencies, ensuring integrated and effective service delivery.
- Promote independence and resilience while supporting well-being and safeguarding.
- Co-produce solutions with service users to reflect the personal goals of the individual.
- Maintain records, track outcomes, and provide reports in line with service objectives.
About You
- Have knowledge of, or lived through some of the complex challenges residents face
- Be empathetic, friendly and have a can-do attitude
- Demonstrate a strong commitment to equality, fairness, and safeguarding
- Possess excellent interpersonal and communication skills
- Be skilled in multi-agency collaboration and delivering person-centred support
- Have knowledge of PIE and trauma-informed approaches
- Be willing and able to work at any of our sites across Birmingham
