Community Reablement Assistant from £25,989 to £27,254 per annum pro rata Bournemouth, Christchurch, Poole (BCP) Permanent Full-Time Join Our Team as a Community Reablement Assistant helping people in the community to gain independence again and Make a Real Difference to People’s Lives! About the role: Join a dynamic and supportive therapy led team and embark on a fulfilling journey as a Community Reablement Assistant. In this flexible and rewarding role, you will be working alongside Occupational Therapists and Reablement Officers playing a vital part in helping individuals regain their confidence and independence after a hospital stay. As the first point of contact for those returning home, you’ll provide essential support and bring positivity to their recovery journey, making a meaningful difference in their lives every day. What you’ll do * Supporting people with their personal care and empowering them to do as much as possible on their own. * Monitoring and providing feedback on the progress of people’s goals. * Assisting with medication and encouraging independence. * Completing care records and other relevant paperwork. * Facilitating access to community groups and services. A valid licence and use of a car for work purposes will be required as the role involves visiting people across the Bournemouth, Christchurch and Poole area. 35 hours per week working 5 /7 days including alternate weekends No prior care experience? No problem! We’ll provide all the training and support you need to succeed and grow in your role.
