An exciting opportunity has arisen for an experienced Advanced Clinical Practitioner (ACP) to join our established and forward-thinking clinical team. You will work autonomously within your scope of practice, delivering high-quality, patient-centred care, including assessment, diagnosis, treatment, and management of patients with a wide range of clinical needs. This role offers the opportunity to contribute to service development, clinical leadership, and education within a supportive multidisciplinary environment.

Main duties of the job

  • Perform advanced clinical assessments and formulate differential diagnoses
  • Develop and implement treatment and management plans
  • Independently prescribe medication (where qualified)
  • Manage patients with complex, acute, and chronic conditions
  • Work collaboratively with multidisciplinary teams
  • Support service improvement, audit, and clinical governance
  • Provide mentorship and supervision to junior staff
  • Contribute to education, training, and research activities

Job responsibilities

The successful applicant must be competent in practice using their expert clinical knowledge and skills, with the freedom and authority to act and make autonomous decisions in the assessment, diagnosis and treatment of patients. The role is key to streamline our triage system and an opportunity for patients to avoid the need to be referred to a GP, thereby enabling them to receive timely care and negate unnecessary delay in receiving treatment. The post-holder will work at the level of advanced clinical practice and has developed and can evidence the underpinning competencies applicable to the specialty or subject area, i.e., the knowledge, skills and behaviours appropriate to the healthcare professionals setting and job role. It is also an opportunity for the post-holder to develop close, long-term relationships with their patients and work in partnership with them to achieve optimum health. The successful applicant will have the autonomy to make decisions based on assessment, diagnosis and the interpretation of test results and will initiate, evaluate and modify a range of interventions which may include prescribing medicines, therapies, lifestyle advice and care.

Person Specification

Qualifications

  • MSc Advanced Clinical Practice or equivalent Level 7 qualification
  • Independent Prescriber qualification-V300 or willingness to obtain

Experience

  • Experience in primary care
  • Experience in leading service development or quality improvement initiatives
  • Teaching, mentorship, or supervision experience
  • Demonstrable experience working at an advanced level of practice
  • Experience managing complex patient presentations
  • Experience working within multidisciplinary teams
  • Experience of practice within the four pillars
  • Experience of prescribing and undertaking medication reviews
  • Experience of working in a GP Practice
