We are seeking a dedicated and compassionate Care Manager to oversee the delivery of high-quality care to our service users in the community. The ideal candidate will have a strong background in senior care, particularly in managing care plans for service users with dementia and other health conditions. This role requires exceptional leadership skills, the ability to supervise staff effectively, and a commitment to enhancing the quality of life for our service users .
*Responsibilities*
* Develop and implement individualised care plans and risk assessments tailored to each service users needs, ensuring compliance with relevant regulations.
* Manage daily operations within the office environment
* Conduct regular assessments of service users\’ health and wellbeing, adjusting care plans and risk assessments as needed.
* Foster a positive environment that promotes dignity, respect, and independence among service users and staff .
* Collaborate with families and healthcare professionals to ensure comprehensive care for all service users.
* Maintain up-to-date knowledge of best practices in dementia care and senior living.
* To share on call phone with 3 other colleagues each month
*Experience*
* Experience in developing and managing care plans and risk assessments effectively.
* Excellent supervisory skills with a track record of fostering teamwork among staff.
* Knowledge of medication administration protocols and compliance requirements.
* Exceptional communication skills, both verbal and written, with the ability to engage effectively with service users, families, and team members.
* To either have a Level 5 in Leadership and Management or to be willing to achieve this in 12-18 months
Please note this is not a typical Monday to Friday 09.00-17.00 job, you will be expected to be hands on and work in the community with our Service users when required.
This position is ideal for someone who is passionate about providing exceptional care to seniors while leading a dedicated team in a supportive environment.
