This is an excellent opportunity for someone with strong care-sector experience who is ready to step into a pivotal coordination and leadership role supporting both staff and service users. As a Care Coordinator, you’ll play a crucial part in ensuring that individuals receiving care experience dignity, independence, and consistency throughout their support. You will work closely with the Registered Manager to deliver a well-structured, compliant and person-centred service.

Key Responsibilities

  • Care & Service Quality: Coordinate high-quality home-care packages that meet regulatory requirements and company standards. Assess needs, develop personalised care & support plans, and ensure risk assessments remain current. Maintain accurate digital records and ensure documentation meets CQC expectations. Manage new referrals, accept care packages, and support with fee discussions when required. Investigate complaints, implement corrective actions, and share learning with the wider team. Contribute to delivering care when necessary and participate in the out-of-hours on-call rota.
  • Staff Support & Leadership: Provide supervision, support, and appraisal for care staff. Create staff rotas via the organisation’s scheduling software. Monitor performance, sickness, annual leave and competence assessments. Work alongside the recruitment team to manage staffing needs and attend relevant industry workshops. Lead and participate in staff meetings, audits, and performance monitoring activities.
  • Compliance & Professional Conduct: Ensure the service meets the requirements of the Health & Social Care Act 2008 (Regulated Activities) Regulations 2014. Keep up-to-date with CQC legislation and changes. Promote safeguarding, equality, and safe working practices. Support operational and strategic goals as set by the management team.

Candidate Requirements

  • Level 3 (or above) Diploma in Health & Social Care.
  • Minimum 2 years’ experience in a care/support worker role.
  • At least 1 year’s experience in a managerial, supervisory or coordinator role within a care setting.
  • Strong organisational skills with the ability to multitask and work under pressure.
  • Excellent communication skills—both written and verbal.
  • Confident IT user (Microsoft Office, digital records).
  • Strong people-management ability with a calm, professional approach.
  • Full UK driving licence and access to own vehicle (business insurance required).
