About the Role:
We are seeking an experienced Care Coordinator to join a rapidly growing healthcare provider in the West Midlands. This role offers a mix of office-based responsibilities and field visits, ensuring that clients with complex care needs receive safe, consistent, and high-quality support.
As a Care Coordinator, you’ll oversee care packages, manage staff rotas, and maintain compliance standards while building strong relationships with clients, families, and healthcare professionals. Key Responsibilities:
* Manage and maintain staff rotas to ensure full coverage of care packages.
* Support care plan reviews, risk assessments, and staff supervisions.
* Conduct spot checks, audits, and competency reviews in the field.
* Liaise with clients, families, and healthcare professionals to ensure person-centred care delivery.
* Handle queries and complaints professionally and efficiently.
* Maintain compliance with CQC standards across all operational areas.
* Participate in a shared on-call rota.
About You:
* Previous experience as a Care Coordinator, Field Care Supervisor, or Senior Carer.
* Strong understanding of CQC compliance and person-centred care planning.
* Excellent communication, organisation, and prioritisation skills.
* Ability to work under pressure with a flexible, professional approach.
* Full UK Driving Licence and own vehicle (essential).
