The LYPFT Learning Disability Service provides specialist health services for adults aged 18 and over with a learning disability across community settings, respite services and the Intensive Support Team. Working with service users, carers, Adult Social Care and third-sector and private providers, the service supports people with highly complex needs, including behaviours that challenge, mental health needs, dual diagnosis and complex physical health needs.

An exciting opportunity has arisen for a permanent Band 6 Community Nurse in the West and South Community Learning Disability Team (CLDT). A temporary Band 6 post is also available as a 12-month secondment for internal applicants or a fixed-term post for external applicants. We are seeking a motivated and compassionate nurse to support adults with a learning disability who require specialist input due to vulnerability and complex physical health, sensory, communication and behavioural needs.

Responsibilities

  • Be based within West and South CLDT (Mon–Fri, 9.00am–5.00pm), providing direct clinical care to improve quality of life and prevent avoidable admissions through person-centred care planning.
  • Manage a caseload, coordinate care, provide supervision to junior staff and contribute to service development.
  • Utilise robust support structures including 1:1 supervision with the Senior Nurse Practitioner and CTM, MDT working and community nurse meetings.
  • Undertake comprehensive, person-centred holistic assessments, considering physical and mental health, communication, sensory needs and behaviour.
  • Deliver specialist nursing interventions within community settings and work proactively to reduce health inequalities and prevent avoidable hospital admissions.
  • Develop, implement and review individualised care and treatment plans in partnership with service users, carers and care providers.
  • Collaborate across multi-disciplinary and multi-agency teams (primary care, acute services, mental health services and Adult Social Care) to advocate for the health needs of adults with a learning disability and ensure continuity of care.
  • Provide clinical leadership through supervision and support of junior staff and students, contributing to service development and maintaining high standards of practice.
  • Promote social inclusion, equitable access to healthcare, safeguarding and risk management processes, in line with Trust policies and the NMC Code.
  • Support the Trust as a teaching and learning organisation through training and development activities.
  • Contribute to safeguarding processes in line with local and national guidance.

Qualifications and requirements

  • Qualified and registered with the Nursing and Midwifery Council (NMC) as a nurse; A Band 6 Community Nurse role requires current registration and relevant clinical experience.
  • Experience working with adults with learning disabilities and complex physical health, sensory, communication and behavioural needs.
  • Ability to undertake comprehensive biopsychosocial and health-related assessments and use clinical pathways.
  • Strong ability to manage and prioritise a caseload, demonstrate time management and organisational skills, and provide evidence-based nursing care.
  • Experience in supervising junior staff, students or learners and contributing to service development.
  • Ability to work collaboratively with primary care, acute services, mental health services and Adult Social Care.
  • Commitment to continuing professional development and safeguarding.
  • Knowledge of safeguarding principles and procedures and their application in practice.

It is a teaching trust with links to local universities.
