We are seeking a Support Worker to join our team and provide assistance to individuals in need. The ideal candidate will have a caring nature and a passion for helping others. As a Support Worker for Transitions Housing Solutions, we will provide emotional and practical support to individuals who have a varying range of complex support needs. Our clients have been assessed as individuals that would struggle to live independently because of a mixture of issues such as health issues, emotional problems, or relationship difficulties

Support workers will work with individuals and their families providing practical and emotional support according to the client’s specific needs identified in their person-centred need’s assessments. Typically, our clients include adults with complex support needs, which may include physical health, mental health, learning disabilities and substance misuse needs. THS also provides housing for adult’s ex-offenders that are released with no suitable accommodation and sometimes are struggling with drug or alcohol addictions.

The main role of a support worker is to support our residents to be able to live as independently as possible, by identifying and supporting them in their areas of need. Within this role, it is desirable that the right applicant has experience in working with people with mental health conditions as you will be expected to work with clients who have short term and long-term support needs. The needs of the clients will vary depending on the needs that have been identified during the assessment process.

Within this role support workers and will be required to work with several residents on a day-to-day basis (Mon-Fri) usually on a 1-2-1 basis creating and working through person centred support plans and documenting the support that has been provided so that we can regularly assess the effectiveness of the service provision to ensure the need of the client are being met effectively.

Duties

  • Develop and implement care plans for individuals

  • Utilise IT systems to maintain accurate records

  • Support individuals to appointments or activities

Previous experience in a similar role is desirable

  • Ability to drive and hold a valid driver’s licence

  • Strong communication skills are essential, * GCSE or equivalent (preferred)

Experience:

  • Support Environment: 2 years (preferred)

Licence/Certification:

  • Driving Licence (required)

  • NVQ Level 2 Health & Social Care (preferred)
