About the Role
The Assessment & Referrals Manager is responsible for overseeing the end-to-end referral and assessment process across our services in Northwest England. The role ensures timely, high-quality, and person-centred assessments, leading to appropriate placements that meet individual needs while supporting organisational growth, occupancy, and compliance. You will act as a key link between commissioners, families, and internal teams, ensuring seamless transitions into services and building strong external relationships to maintain a consistent pipeline of referrals.
Key Responsibilities
- Act as the main point of contact for incoming referrals from local authorities, ICBs, case management companies, and other stakeholders
- Manage and coordinate the referral process from initial enquiry through to placement
- Support managers in conducting person-centred assessments covering needs, risks, compatibility, and support requirements
- Produce assessment reports, recommendations, and costings where required
- Ensure referrals meet eligibility criteria and service suitability requirements
- Build and maintain strong relationships with commissioners, local authorities, social workers, care managers, families, and external professionals
- Support occupancy growth by converting referrals into placements
- Work closely with Operations, Housing, Finance, and Quality teams to ensure service readiness
- Maintain accurate referral tracking systems and produce reports on referral activity and outcomes
- Monitor performance against referral conversion rates, occupancy levels, and assessment turnaround times
- Provide performance updates to the Group Relationship Manager
- Ensure assessments comply with CQC standards, the Care Act 2014, commissioning requirements, GDPR, and organisational policies
Key Skills and Qualifications
Essential:
- Proven experience in supported living / health & social care referrals and assessments
- Strong understanding of Adult Social Care frameworks and Commissioning and Funding processes
- Experience conducting complex needs assessments and placement matching
- Excellent communication skills
- Ability to manage multiple referrals in a fast-paced environment
Desirable:
- Experience in business development / occupancy growth
- Knowledge of learning disabilities, autism, and/or mental health services
- Relevant qualification in Health & Social Care or related field
