Job overview
We have an exciting new opportunity to work across the inpatient emergency portals and into the community as an Assistant Practitioner, supporting the new 10-year health plan. You will be key in helping to deliver and develop this pathway to ensure that we support flow and the patient journey appropriately, ensuring high quality care.
Main duties of the job
- You will work in collaboration with the Physiotherapy and Occupational Therapy teams to support and develop the pathways from the emergency portals, out into the community.
- You will have previous extensive experience working with patients within a ward setting and community and will be familiar with processes around these areas.
- You will also be a motivated individual with a desire to inspire and implement positive change which will have a long lasting impact on our overall patient experience and outcomes.
- IT skills are essential as you will be required to use digital systems.

Supporting service users
- Develops, manages, and maintains positive, appropriate relationships with service users, families and carers, demonstrating respect, kindness, compassion and empathy at all times.
- Consistently deploys care, treatment and support strategies to promote and provide person-centred care, including providing information, advice and guidance to service users, carers, families, health professionals and others.
- Understands the social, cultural, and economic influences, individual circumstances, behaviours, and lifestyle choices that impact on health outcomes for service users and their families, including health inequalities.
- Understands the aims and principles of health promotion, protection and improvement and importance of relevant interventions for individual service users and their families, including making every contact count.
- Applies principles of behaviour change within individualised contexts to enable personalised discussion, sensitively communicating complex and/or potentially challenging information to service users if appropriate to facilitate change including through social prescribing.
- Able to identify ways to promote dignity and is aware of individuals’ environments and factors that might cause discomfort.
- Understands a range of organisations in the health and social care system and services provided, referring service users to services appropriately.
Communication and information management
- Understands role requirement and the importance of accessing, maintaining and storing documentation relating to care, in accordance with local guidance and legal requirements for maintaining confidentiality and data protection.

- Communicates through a variety of methods, including the use of interpersonal, written and verbal and nonverbal skills. Exchanges information with service user and families using active listening, compassion, empathy, and coproduction techniques with the service user.

- Contributes to the maintenance of healthy and safe working environments through appropriate practice, use of resources, techniques and behaviours and understanding of relevant legislation.

- Participates in, contributes to, and may lead, audits, service evaluation and quality improvement projects relevant to own work.
