Are you looking for a new challenge whilst working within the Community? We are looking for someone to act as a source of expertise on the assessment and management of patients presenting with a wide range of pathologies to physiotherapists, student physiotherapists, skilled non-registered staff, other healthcare professionals, GPs, Consultants, patients and their carers. Able to provide a high standard of physiotherapy service within Torbay. This will involve community rehabilitation, crisis response and Intermediate care cover. You will be required to work between these areas depending on the needs of the service, which will take part in the Baywide Intermediate Care weekend rota and late duty rota within the zone. This may also include, from time to time, covering other localities within the Trust.
Main duties of the job:
- Deliver high quality care to the housebound patients of Torbay and work as part of a multi-disciplinary team and in partnership with primary care and secondary care.
- Undertake all aspects of clinical duties as an autonomous practitioner, including professional and legal accountability and managing clinical risk for all aspects of own work.
- Some aspects of work will involve delivery of the care as part of a multidisciplinary team. In most cases to be the sole health professional with responsibility for that patient’s episode of care.
- Undertake advanced physiotherapeutic assessment of patients with diverse or complex physical, psychological, cognitive and behavioural conditions in order to provide an accurate diagnosis and prognosis.
- Formulate and deliver individualised physiotherapy treatment programmes, including manual therapy techniques, therapeutic handling, patient education, electrotherapy, provision of exercise (individual/group), and other alternative options.

Detailed job description and main responsibilities:
- Diploma/degree in physiotherapy.
- State registration – current HCPC certificate.
- Working knowledge of a broad range of musculoskeletal, neurological and medical conditions common in the elderly.
- Experience of working in inpatient, outpatient and community settings.
- Carry out assessments and treatments of people with severe physical, cognitive, behavioural and sensory impairment and poor mobility with frequent intense physical effort daily.
Person specification:
- Diploma/degree in physiotherapy.
- State registration – current HCPC certificate.
- Experience within the NHS including experience as a junior in a broad rotational post covering a range of experience in orthopaedics, elderly care, medical, surgical, neurological and musculoskeletal conditions.
- Evidence of CPD maintained in a portfolio, including attendance at recent Postgraduate courses relevant to the clinical field.
- Post graduate training/experience in rehabilitation and elderly care.
Specific Skills:
- An understanding of Clinical Governance and its implications for services, including experience of quality issues and participation in research and audit.
- Knowledge and understanding of complex legislation surrounding health and adult social care.
- Ability to maintain judgement under pressure.
- Ability to organise and prioritise workload.
- Ability to contribute to weekend/evening working rosters.
- Ability to work alone assessing and managing risk.
- Ability to be mobile in the South Devon area to complete community visits and attend meetings.
