An exciting opportunity has arisen for two full time positions as a Cancer Patient Navigator in a progressive Cancer Services Team.
We are looking for a motivated, organised and enthusiastic team player to join an established team. Training will be provided to support a new starter in this role. The individual will have an eye for detail ensuring accurate data input in line with National Cancer Waiting Times Guidance, Quality Surveillance and COSD and good communication skills

This important role is to help support and navigate patients, their families and carers through Cancer pathways ensuring that the service’s prime aim is to be patient-focused and proactive in seeing that the patient receives the very best cancer pathway experience within the shortest time frame with continual communication and support.

Detailed job description and main responsibilities
- Track & chase diagnostic investigations for all patients on specific cancer pathways ensuring these are booked to meet timed pathway requirements. Ensure results are available for Outpatient appointments andMDTs.
- . Proactively liaise with patients on cancer pathway to ensure they are aware of diagnostic pathway expediting investigations whilst meeting their preferences reducing risk of wastedappointments.
- Liaise with GP practices to retract referrals where patients are not engaging in the cancer pathway or inappropriatelyreferred.
- Book post-MDT follow up appointments for certain tumoursites
- Track & chase surgical/tertiary hospital investigations including PETscans
- Attend cancer PTL (Patient Tracking List) meetings for relevant tumoursites
- To carry out administrative and clerical duties as requested within the department in an efficient and confidentialmanner.
- To assist the team in collating excellent website- cancer relatedinformation.
- To communicate with patients families and carers in a sensitive and confidential manner working closely with clinical teams asrequired.
- Use electronic hospital systems to pull of results or information asrequired.
- The post holder will ensure records are maintained on hospital ITsystems
- The post holder will be expected to exercise judgement when dealing with enquiries and analyse information to resolve the patientqueries.
- The post holder will support the team in managing diaries, arranging meetings, scheduling clinics. There is an expectation to plan and organise straight forward activities on an ongoingbasis.

- The post holder will support audits and survey completion within the department gathering data as required.
- The post holder will be required to cover other tumour sites during periods of annual leave.
Person specification
Qualifications
- Educated to at least A-Level/diploma level or equivalent
- Demonstrable evidence of recognised IM&T qualifications (minimum equating to ECDL)
- Demonstrable evidence of further (relevant) Continual Professional Development
- Keyboard skills to RSAII or equivalent
- Database management
- Oncology for non-clinicians and/or National Cancer Intelligence Network elearning cancer modules
Experience
- An understanding of medical terminology relevant to the role
- Able to demonstrate experience of effective working in a team environment (any setting)
- Previous experience of coordination of a service
- Previous experience of using data validation and extraction software
- Previous experience of using MS Office applications
- Experience of handling highly sensitive information
- Previous NHS Experience
- Familiarity with patient clinical notes and data systems
