Stockport Foundation Trust is looking for an enthusiastic, dynamic, experienced Leader who will form part of the Leadership Team within the Directorate of Out of Hospital, Urgent Community Services. The post will be pivotal within the leadership of our Hospital @ Home Service (Virtual Ward) and our newly created Digital Hub. It will work closely with Leads in Urgent Community Response and Advanced Practitioner service. The Directorate Services form part of the Urgent response within the Neighbourhood with prevention of avoidable admission high priority. This role will be instrumental in the transformation projects currently ongoing within the teams as we stride ahead to support our patients to remain in their usual place of residence.

Main duties of the job

  • Pathway lead will have identified services within the Directorate and will ensure the efficient and effective running of the services.
  • Supervise the provision and delivery of the pathway services by working in collaboration with Clinical Leads, Team Lead and team members.
  • Develop and implement operational management systems and process pathways specific for the individual services patients, in collaboration with services within and external to the Organisation.
  • Work alongside the Team Lead and/or Clinical Leads offering critical challenge within current practice, thus ensuring that all care delivered is evidence based, effective and efficient.
  • Ensure that all care provided by the team members is evaluated and reviewed and efficiency maximised.
  • Adopt a LEAN methodology and philosophy.
  • Lead with key stakeholders on the identification, development and evaluation of operational objectives and create the appropriate SOP’s, protocols, and strategies to maintain efficiency and effectiveness.

If successful this vacancy will require an enhanced DBS check at a cost of £54.90. This cost will be deducted from your first Trust salary. In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.

  • Education & Qualification
  • Registered Nurse/Professional
  • Management /leadership qualification or equivalent experience
  • Postgraduate study at advanced level, or equivalent experience
  • First level degree or equivalent experience
  • Coaching qualification or experience
  • Knowledge
    • Understanding of clinical risk and incident management, patient safety and clinical quality improvement initiatives
    • Understanding of research/evidence-based practice
  • Experience
    • Experience of working in senior clinical or managerial role for 2yrs
    • Evidence of significant practical contribution to service development
    • Evidence of managing complex patients, and advanced communication in stressful and emotional situations
    • Evidence of continuous professional development and practical application to work place
    • Extensive experience of working within Community senior role (Band 7 or above)
  • Skills & Abilities
    • Confidence and ability to accurately assess and manage complex patients
    • Documented evidence of advanced clinical skills and competencies
    • IT Literate
    • Confidence to discuss patient management with senior colleagues and adjoining organisations
    • Commitment to working in a multidisciplinary team (both primary and secondary care)
    • Professional and patient focused, with a strong commitment to providing high quality care.
  • Work Related Circumstances
    • Able to remain professional, calm and polite under pressure
    • Flexible and adaptable in approach to work

    You must have appropriate UK professional registration. This post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
