An exciting opportunity has arisen for a motivated and experienced professional to join the Learning Disability Intensive Support Team (LD IST) as Team Manager. The IST provides a specialist community-based crisis and stabilisation service for adults with learning disabilities and/or autism, supporting admission avoidance and high-quality, person-centred care.
The post holder will provide day-to-day operational leadership while working closely with the Clinical Nurse Specialist to maintain strong clinical oversight, safe decision-making and effective MDT coordination. This role is key in supporting service development, quality improvement and shaping the future direction of the IST as it enters a period of positive transformation.
We are looking for a compassionate and forward-thinking leader with experience in LD/autism services, crisis pathways or complex care, who is committed to delivering safe, responsive and high-quality support for service users, families and staff.
Main duties of the job:- Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence and have use of a vehicle.
- The post holder will provide operational leadership and day-to-day management of the Learning Disability Intensive Support Team (IST), ensuring the effective delivery of a responsive, community-based crisis and stabilisation service for adults with learning disabilities and/or autism.
- Although the role is primarily operational, the post holder will hold clinical oversight and work jointly with the Clinical Nurse Specialist (CNS) to maintain high clinical standards, support safe decision-making and ensure robust governance across the service.
- Together, they will build, monitor and maintain service delivery outcomes, ensuring that assessments, risk formulations, PBS-informed interventions and stabilisation plans align with evidence-based practice.
- Ensure robust MDT working, including risk reviews, CTR/CRTs and Blue Light meetings.
- Ensure practice aligns with the Mental Capacity Act, Mental Health Act, safeguarding and statutory responsibilities.
- Ensure interventions are delivered within the least restrictive, community-based environment.
- Promote effective communication with Community LD Teams and inpatient LD/MH units.
- Support admission avoidance and timely discharge through collaborative system working.
- Education/Qualifications: Registered Nurse (LD/MH), Occupational Therapist, Social Worker or Psychology OR significant operational leadership experience in LD/autism services.
- Experience: Experience working with LD/autism; Experience in crisis intervention and behaviour of concern pathways.
- Skills & Abilities: Strong operational leadership skills; Experience developing service pathways or leading QI projects.
- Personal Qualities: Able to remain calm and composed during high pressure or rapidly changing situations; Highly organised with the ability to maintain oversight of service delivery and prioritise effectively; Adapts well to unexpected pressures, shifts in demand and changes in risk; Demonstrates compassion and a commitment to person-centred care; Understands the wider LD and autism system and can influence development within it.
- Other: Able to travel independently across the Trust.
