Swansea Bay Health Board is seeking to recruit a Health Care Assistant to join the Swansea Medicine Management Team in Domiciliary Care. This is an excellent opportunity for a HCSW with experience in the community setting and a good understanding of medicine management to help support individuals in their homes.
Main duties of the job:- Work under the direction of Pharmacy Technicians to provide medication support to individuals in their own homes across Swansea Bay Health Board.
- Have excellent communication skills to ensure close working relationships with everyone involved in the Swansea MAR scheme.
- Support the team with the provision of undertaking delegated tasks associated with Swansea’s MAR scheme.
- Assist the team in undertaking provision of warfarin MAR charts to service users in the domiciliary care setting.
- Take blood samples from the service users and deliver timely to the laboratory for INR testing.
- Communicate effectively with all patients, carers, families and members of the district nursing team and other significant agencies.
- Assist and advise patients, carers and families to promote independence and self-care.
- Meet identified patient needs and organise own workload priorities under the direction of the Team Leader/Caseload Holder.
- Qualifications: QCF level 3 in Health, Phlebotomy training, Medicine Management Competency assessor.
- Experience: Significant experience of working as a Health Care Assistant, experience of working in a community setting, understanding of the Swansea MAR scheme.
- Skills: Ability to work effectively as a team player under appropriate supervision, ability to deal with non-routine and unpredictable nature of the workload, ability to assess the level of support an individual requires with medication.
- Knowledge: Awareness of the importance of patient-centred care and promoting independence, understanding of when to seek advice and escalate to the appropriate professional, understanding of medicine management issues, understanding of caring for individuals with particular conditions.
