We provide joined-up mental health, physical health and learning disability services to people of all ages across Gloucestershire. Our Home First & Reablement service supports people returning home after a stay in hospital, helping them continue their recovery in their own surroundings.
As a Rehabilitation Assistant, you will work closely with other health care professionals, delivering essential clinical and personal care. You will ensure that our service user needs are met and assist the team in maintaining high standards of care delivery.
Main duties of the job:
- Work core hours of 07:00-22:00 on a 4 day on, 4 day off shift pattern.
- Locality based visiting; a valid driving licence for the UK and access to a vehicle is essential.

As a Rehabilitation Assistant, you will be responsible for participating and assisting in the delivery of care/services to patients and service users as part of a Mental Health and Social Care Health Service.
Person specification:
- Qualifications: GCSE level Pass or equivalent in Maths and English, NVQ in a relevant topic or other relevant training, Care Certificate.
- Experience: Working with people, their families and carers; confident in handling IT systems; experience in a health or social care environment providing personal care.
