Job Overview:

Main Duties of the Job:
- To provide rehabilitation support services.
- To undertake specific skilled support work to aid the rehabilitation of patients in the Community.
- To carry own patient caseload, assessing and treating routine patients within guidelines agreed by the treating therapist.
- Monitor progress and make minor alterations to patient treatment programmes as required.
- Work unsupervised, reporting back patient progress and informing the treating therapist of any problems.
Detailed Job Description and Main Responsibilities:
- To gather information prior to seeing clients and provide feedback to the therapist.
- To contribute to the assessment process and provide feedback to the therapist.
- To implement therapy programmes/approaches, both on an individual basis and for groups.
- To monitor progress and implement changes as appropriate, keeping the therapist informed.
- To contribute to the evaluation of therapeutic interventions, including discharge planning.
- To prioritise workload, following managerial supervision.
Person Specification:
- Experience of working with the public in a variety of settings and able to demonstrate good customer care.
- Experience of working in a health care setting, with skills and experience in rehabilitation of patients.
- Good verbal and written communication skills.
- Good organisational skills.
- Ability to work on own initiative and as part of a team.
Education:
- 5 GCSEs at Grade C or equivalent including English language and Maths.
