The Adult Nutrition & Dietetic Service, provides an integrated service across both our Hospital and Community sites meeting the needs of our patients across the whole of the Bolton population. The Diabetes Dietetic team is made up of 2.39 W.T.E Dietitians to provide a reach in service to the Adult Diabetes Service in Bolton (Band 7, band 6, band 4 Dietetic apprentice). We are looking to recruit a Dietitian who wish to develop their skills in supporting patients with Type I and complex Type 2 diabetes. The successful candidates will be part of the Adult Community Dietetics Team based in the centre of Bolton, providing an in-reach service to the adult Diabetes MDT.

Main duties of the job

You will be reviewing patients in a number of settings including the across the Bolton locality. Providing support in maternity and young adult clinics. The successful candidate will input into the diabetes MDT . Primarily the workload will include General Diabetes Clinics, Antenatal Clinics, Young Adult Clinics and some inpatient ward cover for diabetic patients.

As a department we encourage quality improvement and audit work.

You will report to the Diabetes Dietetic Team Leader, the successful post holder will be part of the wider Bolton Foundation Trust Dietetic Department. The department is composed of two teams, acute and community with a total of 19 W.T.E staff. We provide an integrated service across both our Hospital and Community sites meeting the needs of our patients across the whole of the Bolton population.

We are experienced trainers for student dietitians currently providing P1, P2 and P3 placements with the University of Chester, MMU and UCLAN.

The service operates a flexi-time working arrangement and recognises the importance of balancing the needs of work with family commitments. The post is a 37.5 hours/week post although part time hours may be considered.

Detailed job description and main responsibilities

To be professionally and legally accountable for all aspects of own work, including the management of patients.

T o undertake a comprehensive assessment of patients including those with diverse or complex presentations/multi-pathologies; use advanced clinical reasoning and problem solving skills to provide an accurate prognosis of their condition.

To participate in holistic multi-disciplinary assessments in order to determine client needs.

To assess patient understanding of treatment proposals, gain valid informed consent and have the capacity to work within a legal framework with patients who lack capacity to consent to treatment.

To evaluate the effectiveness of treatments and modify accordingly.

To follow clinical risk management processes within own patient caseload.

To represent the team and patients at multi-disciplinary team meetings and case conferences in order to ensure the delivery of a co-ordinated multi-disciplinary service and to integrate therapy intervention into the treatment programme. This will include discussion of patient care, patient progress and involvement in discharge planning.

To give advice and training on intervention programmes on a one-to-one basis, including advice to carers.

To formulate and deliver individual treatment programmes based on a sound knowledge of evidence based practice and treatment options using clinical assessment, reasoning skills and highly specialised knowledge of intervention techniques.

To support the Team Leader and other Highly Specialist / Specialist Dietitian as required.

To work with the patients, carers and colleagues to identify goals as part of intervention plans

Person specification

Qualifications

  • Degree in Nutrition & Dietetics or other relevant degree with PG Diploma in Nutrition & Dietetics
  • Registered with Health and Care Professions Council
  • Member of British Dietetic Association
  • Member of North West & North Wales branch of the BDA.

Experience

  • Recent Experience as a Dietitian
  • Experience of Diabetes

Skills

  • Good communication skills, both written and verbal, with individuals and groups
  • Ability to give formal and informal presentations, and to plan deliver and evaluate teaching sessions.
  • Uses patient-centred approach when working with clients, and making appropriate use of motivational interviewing and dietary counselling skills.
  • Able to work effectively and develop good interpersonal relationships with a range of people, and commitment to multidisciplinary team working
  • Basic IT skills: email, internet, Word, PowerPoint.
  • Good time management and organisation skills, able to work to deadlines.
  • Able to travel around Bolton. The post requires you to have access to a suitable vehicle to be used within your role subject to the provision of the DDA.
  • Full driving license

Knowledge

  • Evidence based knowledge of clinical dietetics for diabetes
  • Knowledge and understanding of Clinical Governance issues, professional code of conduct and confidentiality.
  • Knowledge of NHS priorities, targets, NSF’s and NICE guidance
  • Ability to conduct literature searches and critically evaluate clinical evidence.
  • Knowledge around complex diabetic patients
  • Able to work flexibly, open to new ideas, new ways of working, and modernisation of services.
  • Self motivated and confident to make appropriate decisions, but willing to seek and accept support.
