An exciting opportunity has arisen on Woodland View Ward in the Forest of Dean community hospital for a band 5 registered nurse. The role is on a permanent basis, and the hours are 37.5 per week. We welcome applications from newly qualified individuals.

Woodland View Ward is a 24 bedded inpatient ward that is responsible for the care of patients requiring rehabilitation, sub-acute and end of life care. The medical model is a mixture of Doctor and Advanced practitioners; we also have a multidisciplinary team based on the ward.

You will be responsible for delivering safe and effective nursing care to our service users. You will be expected to take responsibility for your own professional development and will be supported by the Trust to achieve development opportunities as appropriate.

Main duties of the job:
  • Assessing, planning, implementing, and evaluating care for a specific group of patients.
  • Undertaking comprehensive health care needs assessment of patients and reassigning as appropriate.
  • Undertake joint assessments and be responsible for initiating, co-ordinating and implementing discharge planning involving the Multi-disciplinary, Primary care, Mental Health and Social care Teams where indicated, making onward appropriate referrals.
  • Good organisational skills and effective communication skills are essential.
